1. Introduction
Welcome to the Focus Center, we (“we,” “our,” or “us”) provide ADHD diagnostic evaluations for children, teens, and adults. By scheduling an assessment, you (“Client”), agree to these Terms and Conditions, which outline our services, responsibilities, and limitations.
2. Scope of Services
- Our service includes a comprehensive ADHD evaluation, which may involve clinical interviews, standardized testing, and behavioral assessments.
- Upon completion of the evaluation, we will provide a formal diagnosis and a super bill that you may submit to your insurance provider for potential reimbursement.
- We do not guarantee insurance coverage or reimbursement. It is the Client’s responsibility to verify with their insurance provider.
- We do not prescribe medication but can provide referrals to qualified medical professionals upon request.
3. Client Responsibilities
- Clients must provide accurate and complete information during the intake and evaluation process.
- Payment for services must be made in accordance with our pricing and payment policies.
- Clients must follow all pre-assessment and post-assessment instructions to ensure the accuracy of the evaluation.
4. Fees and Payment
- Fees for ADHD diagnostic services are disclosed prior to scheduling an appointment.
- Payment is due at the time of scheduling the service unless alternative arrangements have been agreed upon in writing.
- No refunds will be issued once services have been rendered.
5. Confidentiality and Data Protection
- All personal and medical information is kept confidential in compliance with HIPAA and other relevant privacy laws.
- Client data will not be shared without explicit written consent, except as required by law.
6. Limitations of Service
- Our evaluations are intended for diagnostic purposes only and do not replace a medical or psychiatric consultation.
- We do not provide emergency mental health services. If you are experiencing a crisis, please contact emergency services or a mental health crisis hotline.
- We do not guarantee specific outcomes from the evaluation. Diagnosis and recommendations are based on clinical judgment and standardized criteria.
7. Insurance and Reimbursement
- We do not directly bill insurance providers; however, we provide a Superbill that clients are responsible for submitting to their insurance company.
- The formal diagnostic report we provide is intended to assist with potential insurance reimbursement, but we make no guarantees regarding coverage.
8. Cancellation, Rescheduling and Refund Policy
- Clients must provide at least 48-hour notice for cancellations or rescheduling.
- Late cancellations or no-shows will be subject to a fee as outlined in our cancellation policy.
9. Amendments
- We reserve the right to update these Terms and Conditions at any time. Changes will be effective upon posting on our website or direct communication to Clients.
10. Governing Law
- These Terms and Conditions shall be governed by the laws of California. Any disputes arising from these terms will be subject to the jurisdiction of the appropriate courts in [Your Location].
